How to Schedule an Appointment
New patients may schedule an appointment in one of the following ways:

Request an Appointment Online
Submit an appointment request using our contact form. A member of our team will contact you to confirm your appointment and provide instructions for patient portal access and pre-registration.
Appointment requests are reviewed during business hours. Same-week appointments may be available.
Patient Portal Access
Once your appointment is scheduled, you will receive instructions to complete pre-registration through our secure patient portal.

Pre-registration allows you to:

What to Bring to Your First Appointment
Please bring the following to your visit:

All current medication bottles
-
A copy of your immunization record (if available)

To help streamline your first visit, please complete the appropriate forms prior to your appointment.
New Patient Intake Forms
Patient Consent Forms
Medical Records Release Form
To ensure continuity of care, please complete the Medical Records Release Form, authorizing us to request your prior medical records.
Forms Completion Fees
Our practice receives requests to complete various patient forms, including insurance, disability, FMLA, and other documentation. Completion of these forms requires medical expertise and review of your medical records.
MUST HAVE: Legible Patient Name, Date of Birth, Signature and Date.
We will fill in the form once you provide the doctor or specialist.

Important Notes:

Fees are based on form length and complexity

Payment is required prior to release of completed forms

Requests are processed in the order received

Please allow approximately one week for processing
Medical record requests are priced individually. Please contact our office for details.
These fees are not billable to insurance and are the patient’s responsibility.
Appointment Policies

Arrival Time

First appointment: arrive 30 minutes early

Follow-up appointments: arrive 15 minutes early

Patients arriving more than 15 minutes late may need to reschedule and may be considered a no-show.
Cancellation & No-Show Policy
Appointments must be canceled at least 24 hours in advance.

No-show fee: $35

Fees are not covered by insurance

Fees must be paid before scheduling the next appointment

Two missed appointments may result in dismissal from the practice.

Billing & Financial Responsibility
Insured Patients
We file claims directly with your insurance provider.
Patients are responsible for:

Co-payments

Deductibles
-
Co-insurance

Any non-covered services

Co-payments are due at the time of service. A $5 billing fee applies if billed later.
Self-Pay Patients (Effective 2026)

New patient visit: $150
-
Subsequent visits: $85

Additional labs, procedures, or diagnostics may incur extra costs and will be discussed in advance.

Wellness services are cash-based and must be paid in full at the time of service.
